I confirm that my phone number (or Parents/Guardians phone number), will be added to the relevant WhatsApp group to communicate any relevant Club information such as training updates, team and match information, events etc.

I confirm that I have read, or been made aware of, the organisation’s Club Constitution

I give my consent that if an emergency medical situation arises, the organisation / club may (act in loco parentis if under 16) provide administration of first aid and/or other medical treatment that in the opinion of a qualified medical practitioner may be necessary. I also understand that in such circumstances all reasonable steps will be taken.

I give consent (for myself, or as a parent or guardian of the Club member listed above) for photographs and/or videos taken of me to be used for publication by Trailblazers. Trailblazers are committed to upholding your right to control whether your photograph is taken, and the future use of photographs and/or videos taken of you as part of the Club. You are not obliged to have any photograph or video taken if it against your wishes.  This consent limits the use of these photographs and/or videos to promotion of the Trailblazers Netball Club (including, but not limited to Social Media platforms, Newsletters, Club webpage, Promotional literature, Facebook, Twitter, Instagram, Tik Tok etc.)
